Passport Acceptance Event

The Vermont Passport Agency will be holding a U.S. passport acceptance event at the Akeley Memorial Building in January. 

Appointments required. Walk-in's welcome on January 24, 10:00am - 1:00pm.

PLEASE BRING THE FOLLOWING

1. Citizenship evidence (e.g. birth or naturalization certificate for first-time applicants, or previous U.S. passport for eligible renewal applicants). Original and a photocopy.
2. One passport photo (see examples on travel.state.gov)
3. Appropriate fees. Payable by credit card, check (payable to U.S. Department of State), or money order (exact amount only). *see travel.state.gov for applicable fees.
4. Applicants under 16 will need to be accompanied by one parent/legal guardian and have consent (DS-3053) from non-appearing parent/legal guardian.
5. Applicant 16-18 years old do not require a parent/guardian to apply, but are strongly encouraged to have a letter of consent and copy of one parent/guardians ID.
6. Photo identification (e.g. driver’s license/permit, government or school ID).
7. Complete, but do not sign, the form.
