U.S. Passports

Passport services are available by appointment only. Please call the Town Clerk's office to schedule at (802) 253-6133.

Passport Application – Adult (16+ years old - as defined by the U.S. Department of State)
  • Completed DS-11 form
  • Certified Birth Certificate (or Certificate of Naturalization or Citizenship)
  • Photo identification
  • Check or money order payable to U.S. Department of State – leave dollar amount blank or review the fee schedule carefully at www.travel.state.gov.
  • Check, cash, credit card (3% service fee applies) for payment to Town of Stowe ($35 application plus $15 photo fee)

An application is required for a first-time applicant or for an adult whose passport was lost, stolen or extensively damaged, or has been expired for more than 5 years. 

Passport Application – Minor (under 16 years old - as defined by the U.S. Department of State)
  • Completed DS-11 form
  • Certified Birth Certificate
  • Parents listed on the birth certificate must be present with photo ID or the accompanying parent may submit:
    • Completed, notarized, DS-3053 for absent parent or
    • Completed DS-5525 or
    • Final court order confirming exclusive parental rights
  • Check or money order payable to U.S. Department of State – leave dollar amount blank or review the fee schedule carefully at www.travel.state.gov.
  • Check, cash, credit card (3% service fee applies) for payment to Town of Stowe ($35 application plus $15 photo fee)
Passport Renewal – Adult only

  • Completed DS-82 form
  • Current passport (valid or expired fewer than 5 years)
  • Photo
  • Check or money order payable to U.S. Department of State – leave dollar amount blank or review the fee schedule carefully at www.travel.state.gov. You are responsible for mailing it.
    Important Notes
    • Fill and print out forms online.
    • Other forms may be required based on your circumstances.
    • Fees vary depending on product (passport – passport card) and processing (routine – expedited).
    • A Learner’s Permit or Driver’s License that reads “Not for Federal ID” must be accompanied by secondary form of photo ID.
    • Photos not taken during your appointment must be current and meet all requirements.
